All accidents and near-misses should be investigated, though the depth of investigation should be proportionate to the actual or potential severity. Serious incidents require thorough investigation; minor incidents may need only brief review. The key is to identify and address causes before a more serious incident occurs.
What is a RIDDOR reportable injury?
Specified injuries that must be reported include fractures (except to fingers, thumbs and toes), amputations, serious burns, loss of consciousness, and injuries requiring hospital treatment for more than 24 hours. Any injury causing more than 7 consecutive days off work must also be reported.
