Accident Investigation: Your Questions Answered

When should an accident be investigated?

All accidents and near-misses should be investigated, though the depth of investigation should be proportionate to the actual or potential severity. Serious incidents require thorough investigation; minor incidents may need only brief review. The key is to identify and address causes before a more serious incident occurs.

What is a RIDDOR reportable injury?

Specified injuries that must be reported include fractures (except to fingers, thumbs and toes), amputations, serious burns, loss of consciousness, and injuries requiring hospital treatment for more than 24 hours. Any injury causing more than 7 consecutive days off work must also be reported.

How quickly can you respond?

We understand that time is critical after an incident. We can provide immediate telephone guidance and, for serious incidents, can usually attend site the same day or next day. Call us as soon as possible after an incident occurs.

What is root cause analysis?

Root cause analysis goes beyond the immediate cause of an incident to identify the underlying failures in systems, processes, or culture that allowed it to happen. By addressing root causes, you prevent not just this incident recurring, but other incidents with similar underlying causes.

Should we wait for the HSE before investigating?

No. You should begin your own investigation promptly while evidence is fresh. If the HSE is investigating, we can advise on how to conduct your internal investigation appropriately and how to work constructively with the HSE inspector.
